Occupational Therapist (OT) Home Health - Full-Time & PRN

TheraCare is seeking a licensed Occupational Therapist (OT) for a Full-Time or PRN home health opportunity providing in-home physical therapy services to adult and geriatric patients throughout Baytown, TX and surrounding areas.

Job Responsibilities

• Perform home health occupational therapy evaluations and re-evaluations for adult and geriatric patients
• Develop and update individualized Plans of Care (POC)
• Treat patients with orthopedic, neurological, cardiac, post-surgical, or other medical conditions
• Address ADLs, IADLs, coordination, strength, balance, and functional mobility
• Apply manual therapy techniques and non-complex modalities, including heat, cold, electrical stimulation, and other physician-ordered treatments
• Implement interventions for fall prevention, home safety, and environmental modifications
• Train patients and caregivers in adaptive equipment and durable medical equipment (DME)
• Fabricate splints and recommend assistive devices as appropriate
• Complete accurate and timely documentation in the EMR
• Communicate with physicians and interdisciplinary care teams
• Supervise COTAs and home health aides, as applicable
• Follow all home health regulations, infection control, and safety standards
