Occupational Therapist (OT) - Home Health

Full-Time Field-Based Patient-Centered Care

What You'll Do

As a Home Health Occupational Therapist , you will play a critical role in helping patients regain independence, improve daily function, and safely navigate life at home. You'll partner closely with patients, families, and interdisciplinary teams to deliver personalized, high-impact care.

Patient Care & Clinical Impact
  • Evaluate patients' functional status, including mobility, strength, coordination, communication, and ability to perform daily activities
  • Assess home environments to identify safety risks and recommend modifications or adaptive equipment
  • Develop individualized treatment plans with clear, measurable goals focused on improving independence and quality of life
  • Provide therapeutic interventions to restore function, including retraining in daily living activities
  • Fabricate splints and train patients in the use of adaptive, prosthetic, and orthotic devices as appropriate
  • Support patients returning to work through functional and work capacity evaluations
  • Educate patients, families, and caregivers on treatment plans, safety, and ongoing care needs
  • Collaborate with the care team to coordinate services and ensure seamless, patient-centered care
  • Participate in discharge planning to support successful transitions and continued progress
Care Coordination & Communication
  • Document patient care accurately and timely within the electronic medical record (EMR)
  • Communicate plans of care with physicians and interdisciplinary team members
  • Participate in case conferences, team meetings, and ongoing clinical collaboration
  • Maintain strong, professional relationships with patients, families, and care team members
Leadership & Oversight
  • Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations and company policy
  • When therapy is the sole skilled service, provide oversight of home health aide care as required

Requirements
  • Graduate of an accredited Occupational Therapy program
  • Current registration with the National Board for Certification in Occupational Therapy (NBCOT)
  • Current state licensure as an Occupational Therapist
  • Minimum of one (1) year of clinical experience preferred (home health experience a plus)
  • Current CPR certification
  • Valid driver's license, reliable transportation, and auto insurance
