Occupational Therapist OT Home Health (Prn)

Empath Home Health, a member of Empath Health is seeking a compassionate PRN Home Health OT Occupational Therapist to work with us to provide individualized care in Lee county (Fort Myers, Cape Coral, Lehigh Acres, Estero, Bonita Springs, and surrounding areas).

Locations: home health visits Lee county (including Fort Myers, Cape Coral, Lehigh Acres, Estero, Bonita Springs, and surrounding areas)

Schedule: PRN; 5-15+ home health visits per week

What You'll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled occupational therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need

• Active Florida OT license
• Minimum of 1 year experience as a licensed Occupational Therapist (OT).
• Home health care experience preferred.
• Strong assessment, critical thinking, and patient education skills
