Occupational Therapist (OT) – Home Health

Schedule: Monday–Friday | 8:30 AM – 5:00 PM

Kettering Home Care is seeking a licensed Occupational Therapist (OT) to provide skilled home health services that help patients regain independence, improve daily functioning, and achieve their recovery goals in the comfort of their own homes.

Key Responsibilities

• Evaluate patients and develop individualized occupational therapy treatment plans
• Provide skilled OT services as ordered by the attending physician
• Monitor patient progress and adjust treatment plans as needed
• Improve strength, mobility, coordination, and functional independence
• Educate patients and caregivers on safety, disease management, adaptive techniques, and equipment use
• Help reduce rehospitalizations through patient and family education
• Complete timely and accurate clinical documentation in compliance with agency, Medicare, and federal guidelines
• Collaborate with physicians and interdisciplinary team members to support positive patient outcomes
• Supervise and provide clinical guidance to COTA team members
• Communicate patient progress and supervisory visit findings with COTA staff
• Partner with COTA professionals to ensure quality care and continuity of services

Qualifications

• Current Occupational Therapist (OT) license in the state of employment
• Minimum one year of OT experience in an acute care setting
• Home health experience preferred
• Valid driver's license and current auto insurance
• Strong communication, organizational, and patient care skills
• Ability to meet the physical demands of the position
