LeeKO Home Health LLC provides exceptional home health services, helping patients regain independence and improve quality of life. We are seeking a skilled Occupational Therapist (OT) and Occupational Therapist Assistant (COTA) to join our team.
Responsibilities OT:
Deliver in-home occupational therapy per physician orders.
Develop treatment plans to improve daily living skills.
Educate patients and caregivers on exercises and mobility.
Document visits in Synergy EMR.
Collaborate with interdisciplinary team members.
Responsibilities COTA: (COTA)
Support OT in patient care plans.
Document progress accurately.
Strong documentation skills.
Document visits in Synergy EMR.
Requirements
Qualifications OT:
Florida OT license.
1+ year clinical experience (home health preferred).
Strong communication, documentation, and organizational skills.
