CURRENT CALIFORNIA OCCUPATIONAL THERAPIST LICENSE
CPR (BLS FOR HEALTHCARE PROVIDERS) CERTIFICATION
CURRENT TB AND PHYSICAL (WITHIN 1 YEAR) ALSO REQUIRED
