We are currently seeking an experienced: OCCUPATIONAL THERAPIST Work with a multidisciplinary team to ensure a wonderful rehab experience for patients and their families by being responsive and individualizing treatment plans. Thrive in a supportive rehab environment with progressive treatment modalities to address a variety of diseases and impairments. In-house therapy with local management support. JOB SUMMARY: Respond to requests for service and obtain physician orders prior to patient evaluations. Evaluate patients and establish individualized, written plans of care featuring objective, measurable, and functional goals. Assess treatment effectiveness regularly and modify plans of care as clinically indicated. Establish restorative and/or maintenance programs tailored to individual patient needs. Complete special requested patient screens as needed. Plan, administer, and supervise therapy programs, including the supervision and training of Certified Occupational Therapy Assistants (COTAs) and OT aides. Communicate effectively with referring physicians, interdisciplinary rehab team members, and facility staff regarding total treatment programs. Maintain accurate, timely, and complete daily treatment records and weekly progress notes in accordance with documentation guidelines. Follow all established Policies and Procedures of Liberty Long Term Care. Submit all required corporate employee paperwork in a timely manner. Maintain a patient workload compatible with the work setting, upholding a productivity rate of at least 75% (or higher as deemed necessary). Prepare, organize, and maintain the work environment, therapy equipment, and supplies. Conduct facility-requested in-services as requested. Travel to other locations to provide therapy services as needed. JOB REQUIREMENTS: Graduate of an accredited Occupational Therapy program. Must meet the criteria of the North Carolina Board of Occupational Therapy. (New graduates must be registered with the National Board for Certification in Occupational Therapy.) Prior experience in a Skilled Nursing Facility (SNF) or long-term care setting preferred. Strong clinical evaluation skills, effective communication, sound time management, and proficiency with healthcare documentation platforms.
