Occupational Therapist (OT)

AvaMed Workforce is a Healthcare Staffing Agency that staffs healthcare providers thought the state of CA. A growing Skilled Nursing Facility in Tulare, CA is seeking a dedicated and compassionate Occupational Therapist (OT) to join the rehabilitation team. This is a full-time weekday position offering a consistent schedule and supportive work environment.

Position Details:
40 Hours per week
Location: Tulare, CA
Schedule: Monday-Friday 8:00am-4:30pm
Rate: $75/hr ($3,000/weekly) W-2 Traveler or $70/hr ($2.800/weekly) W-2 Local 

Job Description:

• Perform occupational therapy evaluations and develop individualized treatment plans based on patient needs and physician orders
• Provide skilled therapy services focused on improving strength, mobility, coordination, and independence with daily living activities (ADLs)
• Implement therapeutic exercises, activities, and adaptive techniques to help patients regain functional abilities
• Educate patients and caregivers on treatment plans, safety precautions, and home exercise programs
• Monitor patient progress and adjust treatment plans as needed to achieve optimal outcomes
• Maintain accurate and timely documentation in compliance with facility, state, and Medicare guidelines
• Collaborate with physicians, nursing staff, and interdisciplinary team members to coordinate patient care
• Participate in care planning meetings and discharge planning as needed
• Ensure a safe and professional therapy environment while following all facility policies and infection control procedures
• Provide compassionate, patient-centered care focused on improving quality of life and maximizing patient independence
