Responsibilities
- Evaluate and develop individualized occupational therapy treatment plans
- Help patients improve skills for daily living, self-care, and functional independence
- Provide interventions for patients with physical, cognitive, or psychosocial challenges
- Collaborate closely with PTs, SLPs, nursing staff, and families for holistic care
- Document progress accurately and timely to support quality care and compliance
Qualifications
- Licensed Occupational Therapist (OT) in the state of [Insert State] or ability to obtain license
- Experience in post-acute or SNF setting preferred, but new grads are welcome
- Strong communication, collaboration, and problem-solving skills
- Passion for patient-centered rehabilitation
