This position gives you access to 13 week placements around Indiana with the flexibility to create your schedule.

Duties

  • Conduct comprehensive evaluations of patients’ physical, cognitive, and sensory abilities to develop tailored care plans.
  • Assist patients with Activities of Daily Living (ADLs), including dressing, grooming, feeding, and mobility tasks.
  • Implement therapeutic interventions aimed at improving motor skills, coordination, and functional independence.
  • Collaborate with multidisciplinary teams to coordinate care plans and ensure seamless patient care transitions.
  • Provide discharge planning support by assessing needs for ongoing therapy or community resources.
  • Work with individuals with disabilities or developmental conditions such as autism and support early intervention programs.
  • Document patient progress meticulously and adjust treatment strategies accordingly to optimize outcomes.

Experience

  • Skilled nursing facility experience or post-acute care background is highly valued for managing complex cases.
  • Experience working with diverse populations including pediatrics in outpatient clinics or school environments.
  • Knowledge of occupational health practices and working with individuals with disabilities across various age groups.
  • Case management skills to coordinate services effectively for patients transitioning between inpatient, outpatient, or home health settings.
  • Strong understanding of medical documentation standards and the ability to develop clear care plans aligned with patient goals.
