The organization has developed new ways to improve rehab services, outcomes, communication, and reimbursement management, helping long‑term care operators capitalize on clinical and financial opportunities. We are seeking a dedicated OccupationalTherapist to join this progressive team and provide person‑centered services in a senior living setting.

Responsibilities

• Evaluate, diagnose, and treat adult and geriatric residents with physical, cognitive, and functional limitations in a SNF/LTC environment.
• Develop and implement individualized, goal-driven treatment plans in collaboration with nursing, PT, SLP, and medical staff.
• Complete timely, accurate documentation of evaluations, daily treatments, progress notes, and discharge plans per facility, payer, and regulatory standards.
• Educate residents, families, and staff on adaptive techniques, equipment use, and energy conservation strategies.
• Participate in care conferences, QI initiatives, and program development to support strong clinical and operational outcomes.

Qualifications

• Current, unrestricted Occupational Therapist license in the state of practice (or eligibility prior to start).
• NBCOT certification preferred; strong new grads with relevant LTC/acute rehab experience may be considered based on client need.
• Prior experience in skilled nursing, post-acute rehab, or long-term care strongly preferred.
• Excellent communication, documentation, and collaboration skills; resident-centered, outcomes-focused approach.

Compensation and benefits

The hiring facility offers a competitive hourly rate (market range in nearby northern Minnesota markets is typically mid-to-high 40s per hour, depending on experience) plus a comprehensive benefits package. Full-time benefits include:
