Victorian Post Acute is a beautiful 90-bed, rehab-focused skilled nursing facility serving both short-term rehabilitation and long-term care residents. We are seeking a compassionate and motivated PRN Occupational Therapist (OTR) to join our dedicated in-house therapy team.

Key Responsibilities

  • Evaluate residents and develop individualized occupational therapy treatment plans in collaboration with the referring physician and interdisciplinary team.

  • Provide skilled occupational therapy services according to the established plan of care.

  • Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with California practice guidelines.

  • Train and support Restorative Nursing Assistants as appropriate.

  • Participate in interdisciplinary care conferences, rehabilitation meetings, and discharge planning.

  • Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.

  • Educate residents, families, and caregivers on home exercise programs, adaptive equipment, and safety recommendations.

  • Recommend and coordinate appropriate durable medical equipment (DME) to maximize resident independence and safety.

  • Report equipment concerns promptly to maintain a safe therapy environment.

Qualifications

  • Current California Occupational Therapist (OTR) license in good standing.

  • Strong communication and collaboration skills.
