What You'll Do
Conduct occupational therapy evaluations and provide treatment within patients’ homes
Create personalized care plans designed to promote independence
Instruct patients and caregivers on safety techniques, adaptive methods, and equipment use
Complete documentation in a timely and accurate manner
Share updates on patient progress with the care team across disciplines
Ensure adherence to all clinical guidelines and documentation requirements
What We're Looking For
Active Occupational Therapist (OT) license
Prior home health experience is a plus, but not mandatory
Excellent verbal and written communication abilities
Capable of working autonomously while effectively managing your schedule
Dependable, professional, and committed to delivering patient-centered care
