Occupational Therapist - Home Health

Location: Polk County, FL, with a primary focus on the Auburndale area
Schedule: Full-Time or Per Diem
Compensation: $100,000-$110,000 total compensation for Full-Time, including base salary and bonus potential | Per Diem rates negotiable

About the Role

Our client is a well-established, Medicare-certified home health organization providing skilled clinical and therapy services to patients throughout Florida. They are seeking a compassionate and motivated Occupational Therapist (OT) to join their growing therapy team and provide in-home care throughout Polk County, with a primary focus on Auburndale and the surrounding communities.

This is an excellent opportunity for an Occupational Therapist who enjoys working independently while remaining part of a collaborative interdisciplinary care team. Home health experience is preferred but not required, making this an opportunity for therapists interested in transitioning into the home health setting.

Both Full-Time and Per Diem opportunities are available.

Responsibilities

  • Provide skilled occupational therapy services to patients in their homes throughout the assigned territory.

  • Perform comprehensive evaluations to assess functional abilities, limitations, safety concerns, and rehabilitation needs.

  • Develop individualized plans of care focused on helping patients regain or maintain independence with activities of daily living.

  • Provide therapeutic interventions addressing mobility, strength, coordination, balance, cognition, and functional performance.

  • Educate patients, family members, and caregivers on therapeutic techniques, home safety, adaptive equipment, and strategies for greater independence.

  • Evaluate the need for adaptive equipment and recommend appropriate modifications to the patient's home environment.

  • Monitor patient progress and adjust treatment plans based on clinical findings and established goals.

  • Collaborate with physicians, nurses, physical therapists, speech therapists, home health aides, and other members of the interdisciplinary care team.

  • Complete required clinical documentation accurately and within established timeframes.

  • Communicate changes in patient condition or progress to the appropriate members of the care team.

  • Maintain compliance with applicable clinical, regulatory, and organizational standards.

Qualifications

  • Current Occupational Therapist license in the State of Florida in good standing required.

  • Graduate of an accredited Occupational Therapy program.

  • Valid driver's license and reliable transportation for travel throughout the assigned territory.

  • Strong clinical assessment, communication, and documentation skills.

  • Ability to manage a field-based caseload and work independently.

  • Patient-focused approach with strong interpersonal and caregiver education skills.

  • Previous home health experience is a plus but is not required.

  • Experience with geriatric or adult rehabilitation populations is preferred.

Per Diem

Per diem opportunities are also available for Occupational Therapists seeking greater scheduling flexibility. Per-visit rates are negotiable based on the individual candidate's experience and availability.

This position offers the autonomy of field-based occupational therapy with the support and resources of an established home health organization. The territory is concentrated primarily around Auburndale and the surrounding Polk County area, allowing the therapist to develop strong relationships with patients, caregivers, and the local clinical team.

For therapists coming from an outpatient, inpatient, skilled nursing, or rehabilitation environment, previous home health experience is not a requirement. Candidates with the right clinical foundation and an interest in providing one-on-one care in patients' homes are encouraged to apply.
