Per Diem Rates: Negotiable based on experience
Our client is seeking a compassionate and motivated Occupational Therapist (OT) to join their growing home health team serving the Port Charlotte area. This opportunity is available on either a full-time or per diem basis and offers the autonomy to manage your own schedule while making a meaningful impact in patients' homes.
Whether you're an experienced home health clinician or looking to transition into the specialty, you'll be supported by a collaborative team focused on providing exceptional patient care. If you enjoy building lasting relationships with your patients and helping older adults maintain their independence, we'd love to speak with you.
Evaluate patients in their homes and develop individualized occupational therapy treatment plans.
Provide skilled occupational therapy services to improve patients' ability to safely perform activities of daily living.
Educate patients and caregivers on adaptive techniques, home safety, fall prevention, and the use of adaptive equipment.
Assess home environments and recommend modifications to maximize safety and independence.
Monitor patient progress and adjust treatment plans as appropriate.
Collaborate with physicians, nurses, physical therapists, speech therapists, and other members of the interdisciplinary care team.
Complete timely and accurate clinical documentation in accordance with agency, Medicare, and regulatory guidelines.
Maintain a patient-first approach while delivering high-quality, compassionate care.
Current Florida Occupational Therapist (OT) License in good standing.
Valid driver's license and reliable transportation.
Ability to travel throughout the Port Charlotte service area.
Strong communication, organizational, and time management skills.
Ability to work independently while collaborating effectively with an interdisciplinary team.
Home health experience preferred.
OASIS experience preferred, but not required.
Strong geriatric experience is highly preferred.
Current CPR certification is a plus.
