As an Occupational Therapist (OT) in home health care, you'll create personalized treatment plans, teach adaptive skills, and collaborate with a caring team, all with the flexibility you deserve!

    Key Responsibilities
    • Develop and implement individualized care plans
    • Provide therapy services per physician's orders
    • Assess functional status and recommend adaptive equipment
    • Teach new skills or retrain patients in daily activities
    • Maintain accurate documentation and participate in case conferences

    Requirements
    • Licensed Occupational Therapist (OT) in the State of Arizona
    • Graduate of an ACOTE-accredited OT program, eligible for NBCOT certification
    • Minimum 2 years' clinical experience (home health preferred)
    • Current CPR certification
    • Valid driver's license, insurance, and reliable transportation
