Job Overview

Tattnall Health Center is seeking a compassionate and dedicated Occupational Therapist (OT) to join our in-house rehabilitation team. We are hiring Full-Time, Part-Time, and PRN (Per Diem) Occupational Therapists to provide exceptional occupational therapy services in our skilled nursing and subacute rehabilitation setting.

As an Occupational Therapist, you will evaluate residents, develop individualized treatment plans, and provide evidence-based interventions that help residents regain independence in activities of daily living (ADLs), improve functional performance, enhance safety, and maximize quality of life following illness, injury, surgery, or hospitalization.

Working collaboratively with an interdisciplinary team, you will play a vital role in helping residents achieve their highest level of function and independence.

Essential Duties and Responsibilities

  • Perform comprehensive occupational therapy evaluations and develop individualized treatment plans based on physician orders, clinical findings, and resident goals.

  • Provide skilled occupational therapy interventions to improve activities of daily living (ADLs), instrumental activities of daily living (IADLs), upper extremity function, strength, coordination, balance, cognition, visual-perceptual skills, and fine motor abilities.

  • Evaluate and treat residents with orthopedic, neurological, cognitive, and medically complex conditions.

  • Train residents in self-care skills, adaptive techniques, energy conservation, and compensatory strategies to promote independence.

  • Recommend, fabricate, and train residents in the use of adaptive equipment, splints, orthotics, and assistive devices as appropriate.

  • Monitor resident progress, reassess functional outcomes, and modify treatment plans as needed.

  • Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, recertifications, and discharge summaries in accordance with Medicare, Medicaid, state, federal, and facility guidelines.

  • Collaborate with physicians, nursing staff, physical therapists, speech-language pathologists, social services, and other interdisciplinary team members to coordinate resident care.

  • Educate residents, families, and caregivers on home safety, fall prevention, adaptive equipment, therapeutic exercises, and strategies to maximize independence.

  • Participate in interdisciplinary care conferences, discharge planning, and quality improvement initiatives.

  • Maintain compliance with all professional, regulatory, and facility policies and standards.

  • Foster a positive, resident-centered rehabilitation environment focused on clinical excellence and quality outcomes.

Qualifications

Required

  • Graduate of an accredited Occupational Therapy program.

  • Current Georgia Occupational Therapist (OT) license in good standing.

  • Current CPR certification or willingness to obtain upon hire.

  • Strong clinical assessment, communication, and documentation skills.

  • Ability to work effectively within a collaborative interdisciplinary healthcare team.

Preferred

  • Experience in skilled nursing, subacute rehabilitation, long-term care, inpatient rehabilitation, or post-acute care.

  • Knowledge of Medicare documentation and reimbursement guidelines.

  • Experience working with geriatric and medically complex patient populations.

  • Familiarity with evidence-based occupational therapy practices, cognitive rehabilitation, adaptive equipment, and upper extremity rehabilitation.
