    Occupational Therapist – Home Health 
     
    Riverside Home Health & Hospice 
    Southern Oregon | Medford & Grants Pass 
     

    Full-Time | Part-Time | PRN 

    $50–$55/hour

    Our Occupational Therapists help patients regain independence after illness, injury, or hospitalization. 
     
    In this role you will: 
     
    • Evaluate patient functional abilities and daily living skills 
    • Develop individualized treatment plans to improve independence 
    • Teach patients adaptive strategies and new ways to complete daily activities 
    • Identify home safety risks and recommend modifications or equipment 
    • Educate patients and caregivers on safe mobility and self-care 
    • Collaborate with physicians, nurses, and therapy team members 
     
    Your work helps patients remain safely in their homes and return to the activities that give life meaning.

    What You’ll Be Doing 
     
    • Evaluating patient functional status and independence with daily activities 
    • Assessing home environments for safety risks and barriers 
    • Developing individualized treatment plans and therapy goals 
    • Teaching adaptive techniques for daily living skills 
    • Recommending and training patients on assistive equipment 
    • Educating caregivers and family members 
    • Supervising Certified Occupational Therapy Assistants when appropriate 
    • Collaborating with the interdisciplinary home health team

    What Helps You Succeed 
     
    • Graduate of an Occupational Therapy program accredited by ACOTE 
    • Eligible for or certified by NBCOT 
    • Licensed Occupational Therapist in the State of Oregon 
    • Strong communication and organizational skills 
    • Current CPR certification 
    • Valid driver’s license and reliable transportation 
     
    Two years of clinical experience is preferred.
