Job Summary:
Anova Care is seeking a licensed Occupational Therapist (OT) to provide home health services to patients in the South Denver area. The OT will be responsible for patient assessment, treatment, and documentation in a home-based setting.

Responsibilities:

  • Conduct patient evaluations and develop individualized OT treatment plans
  • Provide skilled occupational therapy interventions and patient education in the home setting
  • Monitor and document patient progress and outcomes in the EMR
  • Communicate effectively with patients, families, and interdisciplinary teams
  • Ensure compliance with home health regulations, physician orders, and documentation standards
  • Participate in quality improvement initiatives and care coordination meetings
  • Collaborate with clinical, administrative, and operations teams to support patient care and service delivery
  • Perform other clinical or administrative duties as assigned

Qualifications:

  • Master’s or Doctorate in Occupational Therapy required
  • Current state licensure as an Occupational Therapist
  • 1–3 years clinical experience in home health, outpatient, or rehabilitation settings preferred
  • Strong assessment, treatment planning, and patient education skills
  • Knowledge of home health regulations and documentation requirements
  • Proficiency with EMR systems and Microsoft Office Suite
  • Excellent written and verbal communication and interpersonal skills
  • Ability to work independently and manage a patient caseload
  • Availability during business hours with occasional after-hours/weekend coverage

Indirect Reports:

  • Home Health Aides (HHAs) when supporting therapy plans, if applicable
  • Certified Occupational Therapist Assistant

Schedule:

  • Monday–Friday, 8-hour shifts; weekends as needed
Pay: $95,000.00 - $105,000.00 per year
