Occupational Therapist

A compassionate and skilled Occupational Therapist is needed to join an interdisciplinary team (IDT) focused on providing high-quality, patient-centered care. This role is essential in coordinating and delivering rehabilitative occupational therapy services to support participants in achieving maximum independence and function, both at the center and in-home settings.

Key Responsibilities:

• Conduct comprehensive Occupational Therapy assessments to determine patient needs, preferences, and functional goals.
• Develop and implement customized therapy plans in collaboration with the IDT.
• Deliver and document therapeutic interventions, including:
• Use of devices, techniques, and activities to promote muscle re-education, cognitive function, balance, endurance, and ADLs (Activities of Daily Living).
• Supervise Therapy Assistants, offering training and guidance for Functional Maintenance Programs (FMP).
• Provide education and training to staff, caregivers, and families on occupational therapy techniques.
• Work with the IDT to ensure smooth transitions between care settings (e.g., hospitals, skilled nursing facilities, and home care).
• Participate in end-of-life care coordination and support when needed.

Qualifications:

• Bachelor’s Degree in Occupational Therapy from an AOTA & ACOTE accredited program.
• Minimum of 2 years of occupational therapy experience in a clinical setting (preferably with frail or elderly populations).
• Supervisory experience (preferred) – ability to mentor and develop team members.
• National Board Certification in Occupational Therapy (NBCOT).
• Valid California Occupational Therapy License (or eligibility for licensure).

Compensation & Benefits:

• Base Salary: $100,000 - $120,000 (DOE)
