Join our team as a skilled Occupational Therapist, providing essential home health therapy services to patients on their journey to recovery from injuries, illnesses, or disabilities. Your role will involve conducting evaluations, creating and executing personalized occupational therapy care plans, completing OASIS visits and documentation, and collaborating with multidisciplinary teams to enhance patient independence and safety within their homes.

Key Responsibilities
  • Conduct thorough evaluations and develop tailored OT plans of care.
  • Complete OASIS visits and maintain accurate documentation.
  • Work alongside multidisciplinary teams to promote patient safety and independence.
  • Ensure compliance with Medicare guidelines for home health services.
Minimum Qualifications
  • Master’s or Doctorate in Occupational Therapy from an accredited institution.
  • Valid and unrestricted Occupational Therapist license in Indiana prior to employment.
  • Recent experience in skilled home health settings is essential.
  • Familiarity with HomeCare/HomeBase and OASIS documentation.
  • Possession of a current driver’s license, vehicle insurance, and a reliable vehicle for patient visits.
  • BLS certification may be required based on employer policy.
Preferred Qualifications
  • Experience in supervising COTA care in accordance with company policies and state regulations.
  • Excellent written and verbal communication skills, along with strong organizational abilities.
  • Capacity to work independently and adapt to patient scheduling needs.
Assignment Details
  • Shift hours: Monday to Friday, 8:00 AM to 4:30 PM.
  • Guaranteed weekly hours: 36.
  • Mileage reimbursement for travel between patient visits, from the first to the last appointment.
  • Service areas include counties such as White, Pulaski, Fulton, Cass, Carroll, Miami, and Howard (Tipton).
  • Participation in weekend rotations and availability on observed holidays may be required to meet patient needs.

Provide thorough home health occupational therapy that aligns with patient-centered goals, ensuring timely documentation and effective interdisciplinary communication. Conduct evaluations and OASIS visits, including Discharge, Resumption of Care, and Add-On discipline visits. Engage in branch meetings and case conferences as directed by leadership. Flexibility in scheduling is necessary to accommodate patient care demands.
