Job Summary
Nexus at Wood River is seeking a dedicated Occupational Therapist to join our rehabilitation team in a skilled nursing and post-acute care setting. This role may be offered as Full-Time, Part-Time, or Per Diem, depending on candidate availability and facility needs.
The Occupational Therapist will evaluate, plan, and provide skilled therapy services to help residents improve independence with activities of daily living (ADLs), enhance functional abilities, and achieve their highest level of physical and cognitive performance.
Key Responsibilities
- Perform comprehensive occupational therapy evaluations of residents
- Develop individualized treatment plans based on clinical findings and resident goals
- Deliver skilled interventions to improve ADLs such as dressing, bathing, grooming, eating, and mobility
- Train residents in adaptive techniques and use of assistive equipment
- Collaborate with physical therapists, speech-language pathologists, nursing staff, and physicians
- Participate in interdisciplinary care planning and resident progress meetings
- Document evaluations, treatment notes, and discharge summaries in compliance with facility and regulatory standards
- Educate residents, families, and caregivers on home exercise programs and safety strategies
- Support discharge planning and continuity of care

Qualifications
- Degree in Occupational Therapy (Bachelor’s, Master’s, or Doctorate)
- Active Illinois Occupational Therapist license
- NBCOT certification (or eligibility)
- CPR certification (commonly required in SNF settings)
- Prior experience in skilled nursing, rehab, or acute care preferred
- Strong communication, documentation, and teamwork skills
