FOREST GROVE POST ACUTE\n \nOur purpose is to care for people in their moments of need. \n \nOur philosophy of caring is based on our core values and is at the center of all we do.\n \nFGPA is proud of our commitment to excellence and emphasis on quality care guided by a compassionate heart.\n \nBenefits:\n \n\u2981 Discounted employee meals\n \n\u2981 401(k) \n \n\u2981 Employee Assistance Program\n \nGeneral Purpose \n \nThe Staff Occupational Therapist Assistant assists the Staff Occupational Therapist with patient related activities and direct patient care. \n \nEssential Duties \n\n Treat patients as directed by the Occupational Therapist.\n Record daily treatment notes and weekly progress notes per OT Board.\n Treat patients per the physician treatment plan.\n Assist nursing department with training of Restorative Aides.\n Communicate with supervisor and other health team members regarding patient progress, problem and plans.\n Participate in in-services training program for other staff in the facility.\n Record evaluations, daily treatment notes, weekly progress notes, recertification documentation, per Occupational Therapy Board State Practice Act and governmental and third-party payer requirements.\n Record treatment changes per policy and procedures.\n Instruct patient's families or nursing staff in maintenance program and caregiver training in preparation for discharge from therapy services.\n Participate in discharge planning.\n Assist with cleaning and maintenance of treatment area.\n Report any problems with department equipment so that it is maintained in good working order.\n* Ability to relate positively, effectively, and appropriately with patients/residents, families, staff and professional colleagues.\n \nSupervisory Requirements \n \nThis position is not a supervisory position. \n \nQualification \n \nEducation and/or Experience \n \nLicensed as a Staff Occupational Therapy Assistant Board of the State.\n \nProficient in computer skills such as inputting data into programs such as Casamba and Point Click Care. \n \nLanguage Skills \n \nAbility to read technical procedures. \n \nAbility to read and comprehend policy and procedure manuals. \n \nAbility to effectively present information and respond to questions from managers, coworkers and families. \n \nMathematical Skills \n \nAbility to apply concepts such as fractions, percentages, ratios and proportions to practical situations.\n \nReasoning Ability\n \nAbility to solve practical problems. \n \nAbility to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.\n \nCertificates, Licenses, Registrations \n \nLicensed as an Occupational Therapist Assistant in the state.\n \nEmployee must meet continue education requirements per state practices. \n \nMust maintain a license in good standing at all time with the state board.\n \nPhysical Demands \n \nThe essential functions of this position require the following physical abilities: Standing and /or walking very frequently. Sitting occasionally. Reaching with hands and arms frequently, pushing/pulling very frequently. Talking and /or hearing very frequently. Tasting and /or smelling very frequently. Exerting in excess of 100 pounds of force occasionally, and or excess of 50 pounds of force frequently, and/or excess of 20 pounds of force constant to move objects. Climbing, balancing, stooping, kneeling, crouching or crawling occasionally.
