Job Summary and Qualifications

Job Summary:

Assists with the coordination of patient centered and occupational based goals to ensure diagnostic and therapeutic services and rehabilitation are provided to patients. 

  • Accurately follows established treatment plan to set measurable and achievable goals to reach maximum functional independence for Activities of Daily Living (ADL's) and Instrumental Activities of Daily Living (IADL's).
  • Accurately records and documents care and changes patient condition. 
  • Records, monitors, and reviews clinical data. Provides feedback to Occupational Therapist regarding possible modifications to treatment plan and goals based upon patient performance. Within scope of practice and under the direction of the Occupational Therapist adapts recognized therapeutic techniques and modalities to meet the needs of the patient. 
  • Collaborates with patient service departments and community service agencies to coordinate and conduct educational outreach programs and learning activities. Utilizes appropriate methods and material for effective learning, monitors response to education and adapts program as needed. 
  • Supports hospital initiatives in meeting core measures, patient safety and service excellence goals. 
  • Participates in rounding and proactively responds to patient needs to improve patient outcomes and positively impact overall patient experience. 
  • Contributes to a culture of patient centered care by ensuring a quiet healing environment. 
  • Communicates and interacts with patients, families and others in a caring, empathetic, courteous, and respectful manner. 
  • Within scope of practice, communicates clearly and provides explanation of care in a way that can be easily understood by patients and their families. Ensures patient's family is informed of patient status as appropriate. 
  • Communicates effectively and works with physicians and/or other members of the interdisciplinary team in meeting patient needs.
  • Ensures proactive and effective communication is maintained with physician regarding patient observations, treatment responses and accurately documents changes in patient condition or status. 
  • Demonstrates use of the interdisciplinary team approach to patient care; collaborates, confers and updates other team members regarding patient progress, treatment, and discharge planning. 

What qualifications you will need:

  • (OTA) Occupational Therapy Assistant
  • Associate Degree
  • Basic Cardiac Life Support, or BCLS Instructor must be obtained within 30 days of employment start date

HCA Florida Lake Monroe Hospital is a 221-bed acute care hospital. We serve the communities of Seminole and West Volusia County.
