Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, feeding, and meal preparation.
- Provide companion care and supportive assistance to residents
- Promote safety, comfort, and emotional well-being
- Observe and report behavioral or health changes to supervisors
- Communicate effectively with staff, families, and care team
- Maintain a clean, respectful, and supportive environment
- Follow individualized care plans while maintaining client rights and confidentiality (HIPAA compliant).
- Document care and communication clearly and accurately.
- Support mobility, transfers, and safe ambulation as needed.
- Engage clients in meaningful activities, community outings, and social interactions to promote mental and emotional well-being.
- Communicate and collaborate with families, case managers, social workers, and healthcare professionals.
- Document care and communication clearly and accurately.
Skills & Experience
- First Aid & CPR certification (provided if you don't have it)
- Experience in group homes, supported living, or assisted living highly desirable
- Strong observation skills for monitoring behavioral or health changes
- Ability to lift, transfer, and assist with mobility (up to 50 lbs)
- Cooking ability for simple and dietary-specific meals
- Home care, home health, or hospice experience is a plus
- Excellent communication and teamwork skills
Qualifications
- Negative TB test or clear chest X-ray
- LiveScan background check
- Valid driver's license and reliable transportation preferred
