What You'll Do

As an LVN, you will provide direct nursing care to residents while supervising and supporting Certified Nursing Assistants to ensure the highest quality of care is delivered every day.

Responsibilities Include:

Resident Care

  • Administer medications and treatments as prescribed
  • Monitor resident conditions and report changes appropriately
  • Perform skilled nursing procedures including wound care, tube feedings, catheter care, and other treatments
  • Assist with admissions, transfers, and discharges
  • Collaborate with physicians and interdisciplinary team members
  • Ensure resident care plans are followed and updated as needed

Leadership & Supervision

  • Direct and support CNAs and nursing staff during assigned shifts
  • Provide guidance and mentorship to nursing personnel
  • Ensure compliance with facility policies and regulatory requirements
  • Participate in quality assurance and performance improvement initiatives

Documentation & Compliance

  • Maintain accurate and timely nursing documentation
  • Transcribe physician orders and update medical records
  • Complete incident reports and required charting
  • Ensure compliance with HIPAA, infection control, OSHA, and state/federal regulations

Team Collaboration

  • Communicate effectively with residents, families, physicians, and coworkers
  • Participate in shift reports, care conferences, and staff meetings
  • Help create a positive, respectful, and resident-centered environment

Qualifications

Required:

  • Current, active, and unrestricted California LVN License
  • Current CPR Certification
  • Graduate of an accredited LVN program
  • Strong clinical assessment and critical-thinking skills
  • Excellent communication and customer service abilities
  • Ability to work independently and as part of a team

Preferred:

  • Skilled Nursing Facility (SNF) experience
  • Experience with PCC (PointClickCare)
  • Long-Term Care experience
