Summary:
The Oncology Clinical Care Coordinator (OCCC) is responsible for coordinating and managing patient care activities to ensure the delivery of high-quality, patient-centered care. This role involves working closely with patients, families, and the healthcare team to facilitate optimal care and resource utilization. This position will serve as a member of the Oncology Support Services team and will work closely with navigation, social work, and financial navigation. The OCCC will support the front desk/patient access team with clinical matters/questions. This role will report to the Manager of Oncology Clinical Services with a dotted reporting structure to the Manager of Oncology Operations. The OCCC is intended to be patient facing and directly engage in check-out for advanced scheduling (ex: infusions, radiation oncology treatments, injections, imaging, same day multi-service appointments, etc.) This position will be responsible for assisting with incoming and outgoing referrals to include prior authorization needs. In addition, the OCCC is responsible for managing critical clinical work queues and serves as a centralized communication point for providers, clinic nurses, triage nurses and front desk staff to streamline communication for a defined list of clinical items pertaining to the patient care continuum.
