Optometrist

Salary: $60-80+/hr based on experience

LaSante Health Center is seeking an Optometrist to provide comprehensive eye care services in a federally qualified health center (FQHC) setting on per diem basis. This per diem role focuses on delivering high-quality vision care while collaborating with primary care and specialty teams to support overall patient health.

Responsibilities:

• Perform comprehensive eye examinations and vision assessments

• Diagnose and manage common eye conditions and vision disorders

• Prescribe corrective lenses and contact lenses as appropriate

• Identify and refer patients with complex or surgical needs to specialists

• Educate patients on eye health, preventive care, and treatment plans

• Document all encounters accurately in the electronic health record (EHR)

Qualifications:

• Doctor of Optometry (OD) degree from an accredited program

• Current New York State optometry license (or eligibility)

• Experience in outpatient or community health settings preferred

• Strong communication and patient engagement skills
