Home Health Occupational Therapist (OT) – Per Diem
Service Areas: Bucks County

Position Summary: Our client is a a Medicare‑certified and Pennsylvania state‑licensed home health agency is seeking a dedicated PRN / Hourly Occupational Therapist (OT) to provide skilled therapy services to patients in their homes throughout the greater Philadelphia region. Our patients are typically recovering from surgery, hospitalization, illness, injury, or disability, and therapy focuses on helping them safely regain independence in their daily lives. The Occupational Therapist delivers skilled, patient‑centered therapy aimed at improving functional independence and the ability to perform activities of daily living (ADLs). Working independently in the field, the OT collaborates closely with the interdisciplinary care team to support safe recovery, mobility, and long‑term home safety.

Responsibilities

  • Conduct comprehensive occupational therapy evaluations in patients’ homes
  • Develop and implement individualized treatment plans based on physician orders and patient goals
  • Provide therapy focused on ADLs, functional mobility, coordination, strength, and safety
  • Assess home environments and recommend adaptive equipment or home modifications
  • Educate patients and caregivers on adaptive techniques, fall prevention, and home safety strategies
  • Monitor patient progress and adjust treatment plans as needed
  • Collaborate with RNs, PTs, speech therapists, and social workers to ensure coordinated care
  • Maintain timely, accurate, and compliant documentation in the EMR system

Qualifications

  • Active Pennsylvania Occupational Therapist (OT) license
  • Home health experience preferred
  • Strong clinical assessment and patient‑education skills
  • Ability to work independently in a home‑based setting
  • Valid driver’s license and reliable transportation
