Job Summary

We are seeking a dedicated and experienced Occupational Therapist to join our team in a top-tier Skilled Nursing Facility in San Francisco. The OT will be responsible for providing high-quality therapeutic services to residents, promoting independence in activities of daily living, and supporting rehabilitation in a long-term/post-acute care setting.

Key Responsibilities

• Conduct comprehensive OT evaluations and develop individualized treatment plans.

• Provide therapeutic interventions focused on ADLs, functional mobility, cognition, adaptive equipment, and more.

• Collaborate with the interdisciplinary team (nursing, PT, SLP, case management) to optimize resident outcomes.

• Document treatment plans, progress, and outcomes according to facility and regulatory standards.

• Ensure compliance with facility protocols, professional standards, and regulatory requirements.

Qualifications

• Valid California OT license in good standing.

• Minimum 1-3 years of occupational therapy experience in a SNF, long-term care, or rehabilitation setting preferred.

• Strong assessment, documentation, and communication skills.

• Ability to work independently and as part of a multidisciplinary team.

Contract Details & Compensation

• Contract length: Typically 13 weeks (with potential extension).

• Schedule: Monday–Friday, 40 hours/week (as per facility schedule).

• $2,400 – $3,000/week ( $60–$75/hour), negotiable based on experience, shift, and housing/travel stipend eligibility.

• Additional benefits may include travel stipend, housing allowance, and bonus eligibility.
