About the Role: We are seeking a Community Pharmacist to join our team. This role supports patients by improving medication safety during hospital admission, discharge, and in daily life. Responsibilities: Work in the outpatient pharmacy and collaborate with care teams to fulfill hospital discharge orders. Balance hospital discharge needs with the needs of community patients. Rotate as a Medication Reconciliation Pharmacist with other pharmacy staff. Collaborate closely with inpatient and outpatient pharmacy teams and healthcare providers. Work Schedule: MondayFriday: 7:00 AM 5:30 PM. Saturday: 8:00 AM 4:00 PM. Closed Sundays. Rotating holidays and Saturdays. Qualifications: Graduate of an ACPE-accredited School of Pharmacy with a BS or PharmD degree. Licensed to practice pharmacy in Colorado (or must obtain within 60 days of hire). Immunization certified with active BLS card. Skills: Able to work independently with minimal supervision. Adaptable to frequent interruptions and changes in workload. Skilled at setting priorities, making critical decisions, and responding quickly in emergencies. Capable of meeting pharmaceutical needs across all age groups and specialties (neonatal, pediatric, adult, geriatric, psychiatric, oncology, obstetrics, chemical dependency, and rehabilitation). Experience: Community pharmacist experience preferred.M
