The Cardiac Transitional Care Advanced Practice Provider utilizes a patient-centered coordinated care model, demonstrating competencies in leadership, direct clinical practice, consultation/collaboration, coaching/guiding, research, and ethical decision-making. The APN or PA works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs. The position will cover multiple cardiac service lines where needed (Observation, HF/MI, and Cath lab).
- Minimum of 3 years of experience in an acute care setting (hospital-based experience) required
- Cardiology experience strongly preferred
- Current APP licensure and certification (NP or PA) in good standing
- Strong clinical assessment and decision-making skills
- Excellent communication skills, proven team player, ability to multitask and prioritize, and proactive and creative problem-solver with a focus on process improvement, care coordination, and patient-centered solutions.
