Avera Health is seeking a compassionate and motivated Outpatient Physical Therapist to join our Healthcare & Medical Services team. In this role, you will provide evidence-based, patient-centered physical therapy services to individuals across the lifespan in an outpatient clinical setting. You will perform comprehensive evaluations, develop and implement individualized treatment plans, and use therapeutic exercise, manual therapy, functional training, and education to help patients restore function, improve mobility, reduce pain, and enhance overall quality of life.

As part of Avera Health’s collaborative care model, you will work closely with physicians, advanced practice providers, nurses, and other rehabilitation professionals to coordinate care and ensure that each patient receives safe, effective, and timely services. You will regularly reassess progress, modify plans of care based on clinical findings and patient goals, and provide clear communication and education to patients and families regarding diagnoses, treatment options, and home exercise programs.

Daily responsibilities include timely and accurate documentation within the electronic medical record, adherence to regulatory and payer requirements, and participation in scheduling, caseload management, and discharge planning. You will contribute to quality improvement activities, follow evidence-based practice guidelines, and support department initiatives that advance patient outcomes and the patient experience.
