Outpatient Physical Therapy (at-Home Care)

A premier orthopedic group is seeking a motivated and skilled Outpatient Physical Therapist to deliver outpatient-level care directly in patients’ homes. This unique position offers the opportunity to collaborate closely with orthopedic surgeons and the hospital’s management company, ensuring a streamlined, high-quality care experience from surgery through recovery.Duties- Provide evidence-based physical therapy services to post-surgical and orthopedic patients in their homes.- Collaborate with surgeons, physician assistants, and care managers to ensure continuity of care.- Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.- Educate patients and their families about treatment options, exercises, and preventive measures.- Conduct comprehensive evaluations and implement tailored treatment plans- Document all care in alignment with outpatient standards and regulatory requirements- Participate in ongoing case reviews and care team meetings- Work with and supervise PTAsExperience & Qualifications:- Current and valid Physical Therapist license in TX- Minimum 1 year of outpatient orthopedic or home health experience preferred but not required- If new grad, will provide training and mentorship- Strong interpersonal and communication skills- Self-directed, with the ability to work independently in the field- Reliable transportation and willingness to travel within service area- Excellent communication skills for interacting with patients, families, and interdisciplinary teams.What We Offer:- Competitive compensation- Flexible scheduling options- Collaborative environment with orthopedic surgeons- Opportunity to be part of a modern care model bridging outpatient and home health using AIJoin us in making a difference in the lives of our patients through exceptional physical therapy services.
