WHO WE ARE

Kaleidoscope Rx is a neurodiversity-affirming, LGBTQIA+-affirming outpatient psychiatric medication management clinic in Vancouver, WA. We were founded by two women — a PMHNP and an RN — who left toxic workplace environments to build something better: a clinic where patients are treated with unconditional dignity and clinicians are supported to do their best work.

We are a queer, neurodivergent-owned practice. Our founders and much of our team have personal lived experience with ADHD, autism, and neurodivergence. That isn't a marketing angle — it shapes everything about how we practice, what we expect from our providers, and how we show up for patients. Our tagline is "Clear perspective is good medicine," and we mean it.

We serve patients ages 4 and up across Washington and Oregon — adults, adolescents, and children, with a wide range of psychiatric presentations — in person and via telemedicine. Our clinical approach is built on the kind of complex, longitudinal care that requires real clinical skill and genuine respect for patient autonomy.

WHO WE ARE LOOKING FOR

We're looking for a PMHNP who wants to practice with intention — someone who approaches complex cases with curiosity rather than reflexive avoidance, who believes that a great medication management appointment is a collaborative one, and who holds neurodiversity-affirming and LGBTQIA+-affirming care as genuine clinical values rather than checkbox orientations.

This is a psychiatric medication management role. We do not provide primary therapy, and our providers are not expected to function as therapists. What we do expect is that medication management is done thoughtfully — with attention to the whole patient, strong coordination with outside therapists and treatment teams, and a genuine respect for the therapeutic relationships patients have with their other providers.

This role requires experience working with children and adolescents, or a genuine, demonstrated willingness to develop that competency with mentorship and support. We see patients as young as 4, and we need someone who is not only open to that work but committed to doing it well. Clinical discomfort with pediatric populations is addressable — unwillingness to engage with it is not.

We are an LGBTQIA+-affirming practice. We expect all providers to bring that orientation to every patient interaction without exception.

WHAT YOU'LL DO

• Conduct comprehensive psychiatric assessments and deliver individualized, evidence-based medication management across the lifespan, including pediatric patients ages 4 and up
• Prescribe and manage psychotropic medications within PMHNP scope, including for patients with complex histories, polypharmacy, or controlled substance prescribing from prior providers
• Partner with patients — and families, when clinically appropriate — to build treatment plans that reflect their goals, not just clinical benchmarks
• Document clinical encounters in Valant with accuracy and timeliness (follow-up notes within 24 business hours; intake notes within 48 business hours)
• Respond to patient messages, refill requests, and prior authorization requests within 1–2 business days
• Collaborate with the Clinic Lead and Medical Director on complex cases; participate in peer consultation and case review
• Apply neurodiversity-affirming care principles as a foundational clinical orientation — not a specialty add-on

WHAT TO EXPECT — STRUCTURE & SCHEDULE

We've built our clinical model to support providers doing high-quality work without burning out. Here's what that looks like in practice:

• A Mon–Thu, 8am–5:30pm clinic week — we don't work Fridays
• Part-time considered at a minimum of 3 days per week
• 90-minute intake appointments, 30-minute follow-up appointments
• 90 minutes of designated administrative time built into each workday — for documentation, refills, prior authorizations, and case management
• In-person at our Vancouver, WA clinic; telemedicine available for established patients, including Oregon patients seen via telehealth

REQUIRED QUALIFICATIONS

• Master's or Doctorate in Psychiatric Mental Health Nursing
• Active, unrestricted APRN/ARNP license in Washington
• Active Oregon APRN license required prior to start date (we see Oregon patients via telemedicine)
• Active DEA registration
• PMHNP board certification through ANCC (PMHNP-BC) or AANPCB
• Experience with, or demonstrated willingness to develop competency in, prescribing for pediatric patients including children ages 4 and up — training and clinical mentorship provided
• Commitment to neurodiversity-affirming and LGBTQIA+-affirming care as foundational clinical values, not optional orientations

PREFERRED QUALIFICATIONS

• Experience working with children, adolescents, and/or neurodivergent patient populations
• Familiarity with evidence-based psychopharmacology across the lifespan including complex comorbidities
• Experience with collaborative care models and brief psychotherapeutic interventions
• Telepsychiatry experience
• Valant EHR experience, or experience with comparable EHR systems
• Bilingual communication skills

COMPENSATION & BENEFITS

Salary range: $140,000 – $187,000 annually, commensurate with experience and career level.

This is a salaried exempt position. Compensation is tied to our PMHNP Career Levels framework, which provides a structured pathway for growth and advancement based on demonstrated clinical performance.
