Outreach Coordinator - Health Home Program

A health services organization in New York City is seeking a new Health Home Outreach Coordinator to join their team in Brooklyn, supporting community-based outreach, referral development, and enrollment efforts for Health Home programs.

About the Opportunity:

• Shift: Day

• Schedule: Monday to Friday

• Hours: Standard business

• Setting: Hybrid (field-based throughout New York City boroughs with one office day per week)

Responsibilities:

• Build and maintain referral partnerships with healthcare, social service, school, and community organizations

• Conduct field-based outreach to engage prospective members and promote Health Home services

• Support individuals through the enrollment process and maintain accurate referral and outreach records

• Track outreach activity, follow-ups, enrollment progress, and reporting documentation

• Collaborate with internal teams and external partners to support enrollment growth and community engagement

Qualifications:

• At least 1 year of experience in community outreach, enrollment, recruitment, healthcare, behavioral health, social services, or relationship development

• High school diploma or GED required

• Experience with field-based outreach and community visits

• Proficiency with Microsoft Word and Excel

• Experience using an electronic health record system such as EPIC, NextGen, RMA, or Connections

• Strong verbal, written, and interpersonal communication skills

• Ability to work independently while managing multiple priorities and maintaining accurate documentation

Desired Skills:

• Associate's and/or Bachelor's Degree in a related field

• Experience with outreach to children, adults, or behavioral health populations

• Knowledge of Health Home services, Medicaid populations, or community resources

• Experience building partnerships with referral sources and community organizations

• Familiarity with client enrollment, tracking, and reporting workflows.
