JOB SUMMARY:

Ensure significant bilingual outreach and engagement of target population through building and sustaining strong community partnerships and networks. This is a full-time 40-hour week hourly position, utilizing a flex schedule. Moderate degree of travel in community.

  • Conduct community outreach activities aimed at target population through the distribution of materials and information via presentations at local organizations including natural gathering sites, places of worship, and other community-based programs.
  • Identify new community partners and market upcoming events and community-based activities.
  • Support client satisfaction surveys to identify program strengths and areas to improve. Provide feedback to multidisciplinary team based on client observations and interactions.
  • Assist program participants with accessing needed community services (housing, medical, vocational, food, clothing, etc.) through network development, referral and follow up.
  • Accompany participants to appointments, as appropriate.
  • Actively participate as integral part of the CCBHC multidisciplinary team including family care planning and collaboration.
  • Perform basic accountability tasks, include compiling and maintaining all necessary records and assist administrative staff in collection of information for program effectiveness.
  • Participate in all trainings for the Whole Family Health Model.
  • And other duties as assigned.

ESSENTIAL KNOWLEDGE, SKILLS AND ABILITIES:

EDUCATION AND EXPERIENCE:

2 years’ experience in outreach and fieldwork in the behavioral health field. Minimum high school diploma or equivalent. Bilingual strongly encouraged.
