Position Highlights:
Provide attending physician services to a SNF resident panel (typically 75-125 patients)
Conduct admission H&Ps within 48-72 hours, monthly recertifications, and significant-change-of-condition visits
Manage chronic disease, post-acute recovery, wound care, dementia care, polypharmacy, and end-of-life care
Lead and collaborate with NPs/PAs, nursing, therapy (PT/OT/SLP), social work, and consultant pharmacy
Participate in interdisciplinary care planning, family conferences, and Medicare Part A clinical reviews
Document in PointClickCare or facility EMR per CMS PDPM standards
Qualifications:
BC/BE in Internal Medicine, Family Medicine, or Geriatrics
Interest or prior experience in long-term care, sub-acute rehab, or skilled nursing strongly preferred
Active state license (or eligible) and DEA where applicable
Board Certification: yes
