StaffMed Health Partners is recruiting a Post-Acute Care Physician for a leading national physician services organization in a SNF setting in Houston, TX.

Position Highlights:
  • Provide attending physician services to a SNF resident panel (typically 75-125 patients)
  • Conduct admission H&Ps within 48-72 hours, monthly recertifications, and significant-change-of-condition visits
  • Manage chronic disease, post-acute recovery, wound care, dementia care, polypharmacy, and end-of-life care
  • Lead and collaborate with NPs/PAs, nursing, therapy (PT/OT/SLP), social work, and consultant pharmacy
  • Participate in interdisciplinary care planning, family conferences, and Medicare Part A clinical reviews
  • Document in PointClickCare or facility EMR per CMS PDPM standards
Qualifications:
  • BC/BE in Internal Medicine, Family Medicine, or Geriatrics
  • Comfort managing CHF, COPD, diabetes, dementia, palliative needs, and complex polypharmacy
  • Interest or prior experience in long-term care, sub-acute rehab, or skilled nursing strongly preferred
  • Active state license (or eligible) and DEA where applicable
  • Board Certification: yes
