Graduate degree in nursing.
California State License
National Certification, able to see Medicare patients.

National Certification Corporation for Obstetric, Gynecologic and Neonatal Nursing Specialties
Pediatric Nursing Certification Board (previously named the National Certification Board of Pediatric Nurse Practitioners and Nurses)
Oncology Nurses Certification Corporation: ONCC does not assign a certification number but you still need proof they are certified.
AACN Certification Corporation
National Board on Certification of Hospice and Palliative Nurses
Graduate of an accredited nurse practitioner program.
Licensed Registered Nurse in California.
Current documentation of Basic Life Support and First Aid certification is required.
DEA License Required
Minimum of 1 year working with the elderly required.
