Responsibilities:Client Service:
• Conducting assessments
• Developing and implementing person-centered treatment plans
• Coordinating medical and behavioral health services
• Providing crisis intervention including being available for rotating on call shifts for evenings/weekends
• Ability to receive medication management certificate within first 3 months of employment
• Assisting with medication support
• Connecting clients to community resources and advocating for client needs.
• Services are primarily delivered in the community, including clients' homes and other community settings, rather than in an office.
• Willingness to transport clients in agency vehicles and occasionally personal vehicles.
Documentation:
• Complete all documentation within established guidelines.
• Ability to prioritize client need and to complete required written documentation.
• Demonstrate an understanding of all funder and accreditation standards.
• Track and document monthly outcome objectives.
Education:
Bachelor’s Degree in Human services or related field Required.
Experience:
One year of mental health experience required.
License(s)/Certification(s):
Valid driver’s license when driving any vehicle for work-related reasons.
Required to obtain medication management cert within 3 months of hire.
Ability to be CPR/First Aid trained.
Knowledge/Skills/Abilities:
Ability to speak, read and write fluent English.
Ability to read and comprehend common terminology of the mental health field.
Ability to enter documentation in an EHR and use Microsoft Office.
Ability to effectively present information and respond to questions from clients, medical professionals, management, staff and the public.
