Job Description
Responsibilities:
- Conduct home visits for patients requiring pain management
- Perform comprehensive pain assessments and develop individualized care plans
- Prescribe appropriate therapies including medications, injections, and non-pharmacological approaches
- Monitor treatment effectiveness and adjust care plans accordingly
- Educate patients and caregivers on pain management strategies and safe medication use
- Maintain clear documentation in the EMR system
- Collaborate with physicians, specialists, and interdisciplinary teams
Requirements:
- Licensed Nurse Practitioner in the state of Nevada
- DEA license with Schedule II-V prescribing authority
- Experience in pain management, geriatrics, or palliative care preferred
- Strong clinical judgment, compassion, and communication skills
- Reliable transportation and willingness to travel to patient homes
- Bilingual (English/Spanish) is a plus
