Palliative Care Manager (LMSW)

Position Overview
Are you a Licensed Master Social Worker (LMSW) ready to take the next step in your career and make a meaningful impact on patients and families facing serious illness?
We are seeking a compassionate, experienced Palliative Care Manager to help launch and develop a new palliative care program. This is an exciting leadership opportunity for a social worker who wants to help build a program from the ground up while continuing to provide direct patient care when needed.
Reporting directly to the CEO, the Palliative Care Manager will lead program development, oversee daily operations, support staff, and ensure high-quality, compliant care for patients and their families.
Key Responsibilities
Lead program startup activities, including developing workflows, policies, procedures, and operational processes.
Manage the program budget and monitor financial performance.
Supervise and support palliative care staff in collaboration with the Medical Director.
Ensure compliance with Medicare, Medicaid, commercial payer requirements, and accreditation standards.
Assist with provider credentialing and payer contracting.
Oversee scheduling, referrals, clinical documentation, and care transitions.
Drive program growth, customer service, quality improvement, and operational efficiency.
Provide direct social work services during program startup and as needed.
Participate in the hospice Manager On-Call rotation.
Qualifications
Masters degree in Social Work required.
Active LMSW credential required.
Hospice or palliative care leadership experience preferred.
Previous program development or startup experience preferred.
Experience with electronic medical records (EMR) and Microsoft Office applications required.
Strong leadership, communication, interpersonal, and organizational skills.
Ability to collaborate effectively with interdisciplinary teams and lead through periods of growth and change.
