Pay Range: $75,000.00 to $85,000.00/yr. (based on years of experience)

Title: Palliative Care Manager (LMSW)
Location: Grand Rapids, MI 49503
Job Type: Perm
Shift: Mon-Fri 8:30-5 (could be required to participate in manager on call rotation for hospice operations)

Job Description:

  • At Hospice, we understand the role that palliative care can offer in our community and we are excited to bring this service to life alongside our core hospice services. This role offers the rare opportunity to be a cornerstone in launching Hospice’s new palliative care program, shaping its vision, structure, and long-term impact. As a key leader, you will guide the development of innovative care models that will meaningfully support patients who are facing serious illness and their families. Your expertise will directly influence how the program grows, evolves, and becomes an essential part of the Hospice continuum of care.
  • Reporting directly to the CEO, the Palliative Care Manager ensures the program reflects excellence, regulatory compliance, and the mission to support patients and families with serious illness.

Key Responsibilities:

  • Lead all program startup activities, including workflows, policies, and operational design
  • Manage program budget, financial performance
  • Supervise and support palliative care staff in collaboration with the Medical Director
  • Oversee compliance with Medicare, Medicaid, commercial payers, and accreditation standards
  • Support provider credentialing and payer contracting
  • Ensure efficient scheduling, referrals, documentation, and care transitions
  • Drive program growth, customer service, and performance improvement
  • Deliver social work services during start up and as needed
  • Participate in the Manager On-Call Rotation for hospice services

Qualifications:

  • Master's degree in social work required; hospice or palliative care related leadership experience preferred.
  • Prior program development experience preferred
  • Experience in Electronic Medical Records and Microsoft Office products is required
  • Strong interpersonal, communication, and leadership skills with a demonstrated ability to collaborate effectively across disciplines, and guide teams through periods of growth and change.
