Required Qualifications:
- A Bachelor's degree or above.
- A minimum of one year's experience in either Hospice, Palliative Care, or Home Health.
- Must possess current Registered Nurse (RN) and Nurse Practitioner (NP) licenses valid in Illinois (IL).
- Current American Heart Association Basic Life Support or American Heart Advanced Life Support certifications.
- At least five years of practical Clinical Experience.
Preferred Qualifications:
- Be sure to list your valid RN and NP license numbers.
Overview:

Key Responsibilities:
- Orchestrating and executing patient care plans in various environments.
- Partnering closely with the care team.
- Serving the patient community in Lisle, IL, and neighboring regions.
Schedule and Shift details:
- Working hours are Monday to Friday, 8 am to 5 pm with no weekend, holiday, or on-call commitments.

Ideal Candidates Will Have:
- A Bachelor of Science (BSN) degree from an acknowledged Nursing School.
- A Master of Science (MSN) degree from an acknowledged Nursing School.
- At least five years of clinical practice.
- A valid license to practice as a Registered Nurse and Nurse Practitioner.
- Prior experience in community-based Home Health or Hospice/Palliative Care is highly desirable.
