Hours/Status: Full-Time / Non-Exempt; occasional evenings and weekends required
Reports To: Parent Coach Supervisor / Clinical Supervisor
Grant-Funded Position: Yes – contingent upon continued funding

Antelope Valley Partners for Health (AVPH) is a community-focused organization dedicated to improving the health, wellness, and quality of life of individuals and families throughout the Antelope Valley. AVPH partners with local agencies, hospitals, and funders, including First 5 LA, to deliver evidence-based, prevention-focused home visiting programs that strengthen families and promote healthy child development.
JOB SUMMARY
The Parent Coach provides direct prenatal and postpartum home visiting services through the Welcome Baby Hospitals Initiative in alignment with First 5 LA grant requirements. This position is responsible for supporting expectant and new parents through education, assessment, referral, and follow-up services that promote maternal and infant health, early bonding and attachment, and child development.
The Parent Coach supports program documentation, reporting, and data collection requirements necessary for program monitoring and evaluation. Services are delivered in home and community-based settings using a culturally responsive, strengths-based approach.
KEY RESPONSIBILITIES
Direct Service Delivery
Conduct assigned prenatal and postpartum home visits in accordance with Welcome Baby service model requirements
Provide parent education on:
Prenatal and postnatal maternal health
Infant health, breastfeeding, and infant feeding
Parent-infant bonding and attachment
Child development and developmental milestones
Home safety and injury prevention
Build trust and rapport with families using a client-centered, strengths-based approach
Maintain continuity of services for enrolled families within required timelines
Work closely with the Welcome Baby Nurses, other Parent Coaches, Parent Coach Supervisors, Project Manager, and administrative staff to provide a continuum of services.
Screening, Assessment & Referral
Complete required standardized assessments and screenings, as applicable, including maternal depression, attachment, developmental milestones, and home safety
Conduct ongoing informal assessments of family strengths, needs, risks, and barriers
Identify appropriate community resources and referrals based on assessed needs
Provide active follow up to support family connection to referred services
Escalate high risk concerns to supervisory staff in accordance with program protocols
Documentation & Data Management
Complete timely, accurate progress notes for every client interaction
Maintain organized, complete, and compliant client records
Enter individual level client, service, demographic, and outcome data into the Stronger Families Database within required timeframes
Ensure documentation and data entries accurately support:
Services delivered
Client outcomes
Performance targets
Annual Reporting & Data Quality
Ensure documentation and data entry are completed accurately and within required timelines
Support program reporting requirements through accurate client and service documentation
Participate in data quality improvement efforts and respond to requests for documentation clarification as needed
Evaluation, Monitoring & Compliance Support
Participate in program evaluation activities, audits, and data validation efforts as requested
Respond promptly to documentation or data clarification requests
Comply with all confidentiality, consent, and HIPAA-related requirements
Report any suspected confidentiality or data security breaches immediately
Supervision, Training & Team Collaboration
Attend weekly individual reflective supervision and group reflective supervision sessions
Participate in staff meetings, case reviews, and required trainings
Complete all required Welcome Baby, First 5 LA, and agency trainings within designated timeframes
Work collaboratively with Welcome Baby Nurses, supervisors, and multidisciplinary team members
Support program quality improvement and compliance initiatives
KNOWLEDGE, SKILLS & ABILITIES
Knowledge of maternal and child health, early development, and home visiting models
Ability to conduct family assessments using a strengths-based approach
Strong documentation, data entry, and organizational skills
Ability to organize assigned client visits, documentation, schedules, and required follow-up activities
Experience working with culturally diverse and high-risk families
Ability to maintain professional boundaries and confidentiality
Proficiency in Microsoft Office and electronic data systems
TRAVEL REQUIREMENTS
Local travel within the Antelope Valley for home visits, meetings, and trainings
Occasional travel for regional meetings or required trainings
Mileage reimbursement provided in accordance with AVPH policy
REQUIRED QUALIFICATIONS
Bachelor’s degree preferred in Child Development, Social Work, Psychology, Human Development, Public Health, or related field
Associate degree, CDA, or equivalent combination of education and relevant experience may be considered
Minimum one (1) year experience working with families, maternal and child health programs, home visitation, early childhood services, or related community-based programs
Certified Lactation Educator (CLE) or Certified Lactation Counselor (CLC), or ability to obtain within required timeframe
Valid California driver’s license, reliable transportation, and current automobile insurance
Current infant and adult CPR certification
Must pass background clearance, TB testing, and meet health and vaccination requirements
PREFERRED QUALIFICATIONS
Bilingual (Spanish or other languages reflective of the community)
Experience with data systems and grant-funded reporting
Experience working in prevention or early-intervention programs
Familiarity with First 5 LA or home visiting models

Must maintain regular and punctual attendance in accordance with agency standards.
Must comply with all AVPH policies, procedures, and regulatory requirements.
Job duties may be modified to remain compliant with programmatic or grant-related changes.

Local travel within the Antelope Valley region required
