    Classification: Non-exempt
    Reports to: Director of Therapy Services
    Type: Part-Time 10-29 hours per week
    Location: Field-Based

    SUMMARY
    The Community Mentoring Program provides mental health rehabilitation interventions and supports necessary to assist clients in meeting their mental health treatment goals while in his/her own environment. Community Mentors coordinate services with other members of the client’s treatment team such as, therapist and psychiatrist to help clients achieve rehabilitation, resilience, and recovery.

    ESSENTIAL DUTIES AND RESPONSIBILTIES

    Clinical
    • Community Mentors provide community based support to youth, families, or adults with a mental health diagnosis.
    • Support functions provided to clients/families include: teaching social skills, modeling appropriate behaviors, assisting in problem-solving, and psychoeducation.
    • Coordinate services with primary clinician and additional collaterals to help clients achieve optimal recovery.
    • Must maintain and manage a minimum caseload of approximately four to six clients.
    • Provide a minimum of 9 hours a week of direct service to clients. (90% of hours worked per week dedicated to direct service)
    • Provide time-limited intensive supports following a Mobile Crisis Response in order to assist with Crisis Stabilization Services.
    • In the event of a crisis situation when the client is already with the Therapeutic Mentor, staff will continue to provide services until the client is stable, the parent/guardian relieves the Therapeutic Mentor, or other qualified professional (SASS worker, doctor, police, etc.) relieves the worker. This may require working past the scheduled time.
    Supervision
    • Schedule and attend regular supervision with The Manager of Therapy Services to discuss cases and report problem cases, need for special services and resource information.
    • Attend monthly group supervision and participate in Performance Quality Improvement Goals. 
    • Must fulfill agency required training and attend in-service trainings assigned by supervisor.
    Administrative
    • Communicate with the client’s primary clinician/team at least bi-weekly.
    • Timely submission of time sheets every other Tuesday morning to ensure the supervisor is able to review and submit to payroll by Wednesday.
    • Timely submission of reimbursement paperwork for travel expenses on a monthly basis.
    • Complete all other clinical paperwork for cases as needed. This may include Incident Reports, when necessary, DCFS reports, utilization reviews and any requested agency reports as well as any other paperwork clinically necessary for maintaining files.
    • Complete and sign all service notes within 7 days of service delivery.
    • Stay current on Medicaid Rule 132 and Rule 140.
    • Must maintain 100,000/300,000/50,000 automobile insurance. Provide documentation that said insurance is current at the required level when requested by the agency.
    • Must hold a valid, unexpired Illinois driver's license.
    Other Responsibilities
    • Support to clients and their families is often provided in the home or community. Time at the office may be assigned by supervisor to fit program needs.
    • Remain compliant with Council on Accreditation Standards and Best Practices in serving clients
    • Community Mentors will adhere to all the agency policies, guidelines, and deadlines for interventions and paperwork for each case.
    • Responsible for incidental business related travel.
    • As per Family Service Association of Greater Elgin Area all Community Mentors will adhere to the Code of Ethics of the National Association of Social Workers (NASW) when providing services.

    SUPERVISORY RESPONSIBILITY
    This role has no supervisory responsibilities.

    PHYSICAL AND ENVIRONMENTAL CONSIDERATIONS
    • The incumbent will be sitting in car for travel for several minutes every day.
    • The incumbent is frequently expected to walk, sit, stand, bend and drive.
    • The incumbent may also need to climb stairs on occasion.
    • Must be able to lift up to 35 lbs. on occasion.
    • The work environment is mostly field based and will have varying temperatures and noise level.
    • The incumbent can expect to work in client’s homes, schools, public libraries, community agencies, and some in office time.
    Position Type/Expected Hours of Work
    This is a part-time position, 10-29 hours per week, requiring availability to meet the needs of clients assigned to the mentor.
    Hourly Pay Rates
    • In accordance with Illinois state law, we are committed to transparency in compensation. The hourly pay range for this position is $23.92 to $29 per hour, depending on experience and qualifications

    TRAVEL
    This position requires extensive travel throughout Kane and occasionally to areas outside of that.

    EDUCATION AND EXPERIENCE
    • Bachelor’s degree in Psychology, Social Work, or related field is required.
    • Must have reliable transportation as most skills are developed within the community.
    • Afternoon, evening (until 8pm), and weekends are required to meet client’s needs.
    • Must be a good team player, ability to work with people from diverse backgrounds, be flexible, detailed-oriented, and have a positive attitude.
    • Must be able to pass a background check, drug screen, and provide proof of vehicle insurance.
    • Must hold a valid, unexpired Illinois driver's license.
    • Must be 21 years-old.
    • Bilingual in Spanish preferred.
