Part Time Community Outreach Worker - B''more for Healthy Babies

• Functions as a patient advocate through diverse community outreach efforts in East Baltimore

• Identifies potential outreach targets and develops creative and effective ways to reach these targets

• Generates new clients for BMS through contacts with individuals and families currently not connected to primary care

• Educates community members about resources and sources of care, particularly with regard to those intended for mothers and babies

• Works to remove barriers to care by direct contact and relationship building with identified clients and referral agencies

• Collects relevant data and generates reports on outreach activities for use internally and to meet grant maker requirements

• Represents BMS and B'more for Healthy Babies at a minimum of 8 community activities monthly

• Represents BMS and B'more for Healthy Babies on a minimum of one Community Board meeting at least quarterly

• Offers presentations both group and individual on B'more for Healthy Babies educational themes throughout the BHB catchment area in Patterson Park North and East

• Participates monthly in the B'more for Healthy Babies Collaborative for Patterson Park North and East

General duties:

• Consistently and independently follows established safety and infection control procedures. Intervenes in unsafe situations.

• Complies with the standards of conduct and Corporate Responsibility Program and seeks guidance when in doubt.

• Meets or exceeds expectations of individuals for whom service is provided.

• Participates on interdepartmental or departmental committees as appropriate.

• This is a part time, 20 hour/week position.*

Required Education:

High school diploma or general education degree (GED)

Required Certifications, Licenses, Registrations:

Current MD Driver's license and personal vehicle required.

Preferred Certifications, Licenses, Registrations:

Community Health Worker certificate

Required Experience:

1 years' experience in community outreach in Southeast Baltimore area

Preferred Experience:

Knowledge of African American community, organizations, and culture in Baltimore City

This is a part time, 20 hour/week position.

The Community Outreach Worker for B'more for Healthy Babies will engage in community outreach through participation in health fairs, community events and meetings with partner organizations. The Outreach Worker will provide one-on-one education, as well as group presentations, regarding B'more for Healthy Babies themes, such as Safe Sleep, Smoke Free Homes and Access to Care. The Outreach Worker will be familiar with resources available in Southeast Baltimore and assist families in connecting to these resources. Families without access to primary care will be referred to BMS services.

The Outreach Worker will develop and maintain relationships with community organizations in the Patterson Park North and East area in order to disseminate and support the work of B'more for Healthy Babies. Communication skills are required outside of the work group to facilitate work groups, resolve conflict, persuade or negotiate. Represents the company externally and provides information on a variety of programs/services. Carries out responsibilities in accordance with Baltimore Medical System Core Values, policies/ procedures and applicable civil laws. Is accountable for efficient use of resources and support of the BMS mission.
