Your Role as a RN: We are seeking a Registered Nurse (RN) Case Manager to provide skilled nursing care to our home health patients. This position is available aspart-time, with flexible scheduling. Care locations would be in Davis & Weber Counties. You'll work closely with an interdisciplinary team to:

Conduct admission and OASIS assessments Perform head-to-toe and wound evaluations Create and implement individualized plans of care Provide follow-up skilled nursing visits Educate patients and caregivers Coordinate care with MDs, CNAs, PT/OT, and family members Attend bi-weekly team meetings and contribute to care planning

Pay and Benefits

$50 to $105 per visit Mileage reimbursement Flexible schedules available Health/Dental/Vision for full time employees and their families Company paid life insurance for full-time employees 401k available for both part time and full-time employees

Qualifications:

Current Utah RN license Reliable transportation Excellent communication and interpersonal skills

Preferred Qualifications:
