We make house calls – enabling access to quality wrap-around services for homebound older adults with complex health needs while improving the quality of their lives, right at home.
We offer an array of medical and chronic care services and we’re backed by SCAN, a not-for-profit organization that has a history of supporting and improving the health and independence of older adults for over 45 years.
Homebase Medical is expanding rapidly into theWoodland Hills / San Fernando Valley, CA area. We are currently searching for experienced part-timeAdvanced Practice Clinicians (Physician Assistant or Nurse Practitioner)to join our expanding house call practice.
Key Duties:
ConductAnnual Wellness Visitsfocused on patient education
DeliverChronic Care Managementto improve long-term outcomes
Perform comprehensive in-home visits including:
Medication review & chronic illness management
Medical history updates & physical exams
Labs, diagnostic screenings, and post-hospital follow-ups
Primary care management & palliative care services

Qualifications:
ActiveNurse PractitionerorPhysician Assistantlicense in the State of California
Board certification (Nurse Practitioner or Physician Assistant)
DEA licensure
Prior experience preferred (but passion and commitment are just as important
